22例乳腺叶状肿瘤诊治分析

The diagnosis and treatment of phyllodes tumor

目的 分析乳腺叶状肿瘤的病理特征和临床表现 ,探讨诊治方法。方法 回顾性分析 2 2例经病理证实的乳腺叶状肿瘤。结果 行乳房肿瘤扩大切除术 9例 ,乳房切除术 4例 ,乳房改良根治术 2例 ,行乳房肿瘤局部切除术 7例。 4例乳房肿瘤局部切除术后出现复发 ,局部复发率 19%。结论 乳腺叶状肿瘤术前确诊较困难 ,其局部复发与是否手术彻底切除密切相关。对病理诊断为巨纤维腺瘤者 。

Objective To analyze the pathology and clinical situation of phyllodes tumor,and to discuss its diagnosis and treatment method.Methods Analyzed retrospectively 22 specimens which was previously verified as phyllodes tumor on pathological basis.Results 9 cases were treated by amplification exicision of breast tumor,4 cases by mastectomy and 2 cases by nodified radical mastectomy,and 7 cases local excision of breast tumor were made,among which 4 local excision of breast tumor was found recurrent.Local recurrence rate was 19%.Conclusion Its really difficult to make a preoperative final diagnosis of phyllodes tumor.Whether local recurrence will occur or not has close relation to the level of complete excision.If the result of pathological diagnosis turns out to be massive fibroadenoma,more follow up should be made after local excision.
