南京市糖尿病自我管理项目对患者自我效能的影响

Impact of self-management group activities on self-efficacy among diabetic patients in Nanjing city

目的了解南京市糖尿病患者参加自我管理小组活动前后自我效能的改变情况,分析其影响因素,为糖尿病社区防治和管理提供参考依据。方法2015年招募自愿参与的糖尿病患者,通过开展自我管理小组活动进行干预。分别在干预前和干预后6个月,收集基本信息和自我效能得分,采用多重线性回归分析干预效果的影响因素。结果共调查糖尿病患者345例,完成6次小组活动的325例,应答率94.2%;干预后,症状管理及疾病共性管理自我效能、自我效能总分较活动前分别提高(0.89±1.81)、(0.92±1.75)、(5.51±10.29)分,差异均有统计学意义(t值分别为8.55、9.06、9.39,P值均<0.05)。多重线性回归分析显示,以自费方式支出医疗费用、对小组活动总体评价、内容评价非常满意均是自我效能得分增加的正向影响因素(P<0.05)。结论糖尿病患者自我管理小组活动可以提高患者自我效能,值得推广。

Objective To analyze changes in self-efficacy of diabetic patients before and after self-management group activities;to analyze its potential influencing factors;to provide evidences for improving community prevention and management of diabetic patients.Methods The volunteer diabetic patients were recruited to conduct intervention of self-management group activities in 2015.Interviews were carried out before and and 6-month after intervention to collect basic information and self efficacy scores.Multivariate regression analysis was performed to analyze influencing factors of intervention outcomes.Results A total of 345 patients with diabetes were interviewed,among whom 325 patients completed 6 group activities,resulting responding rate of 94.2%.After intervention,symptom management,disease commonness management self-efficacy and total scores of self-efficacy increased by(0.89±1.81),(0.92±1.75)and(5.51±10.29),respectively,with statistical significances(t=8.55,9.06,9.39,all P<0.05).Multivariate analysis showed pay for hospital costs at own expense,overall and content evaluation of great satisfaction were positively associated with self-efficacy score improvement(all P<0.05).Conclusion Selfmanagement group activity intervention could improve self-efficacy of diabetic patients,which can be promoted.
