慢性胃炎脾胃虚、实证候与相关指标的关系初探

An Exploration on the Relationship Between Chronic Gastritis with Spleen and Stomach Deficiency and Excess Syndrome and Relevant Indexes

目的:探讨慢性胃炎脾胃虚、实证候与相关指标的关系,以研究本病的证治规律。方法:观察69例慢性胃炎患者的中医证候分布,同时检测其胃动力学相关指标变化,对中医证候与功能检测指标进行多因素相关性分析。结果:本病早期以实证居多,而虚证患者年龄较大,病程较长,提示中后期以虚证分布较多;实证组餐前正常慢波百分比(Th)与主频率(DF)、餐前主功率(DP)与餐后DF有较强的正相关;而餐前胃动过缓节律百分比(B)与餐前Th、餐前DF与餐前B、餐后B与餐后Th呈负相关:虚证组胃排空率与正常组比较有统计学意义;提示虚证患者存在胃排空率下降,随着消化期餐前胃动过速波节律百分比(T)的增加而下降;餐后DP与餐前DP呈正相关。结论:研究表明中医证候有其独特的客观依据,和慢性胃炎的疾病变化规律相比有其独立性。

Objective: To explore the relationship between chronic gastritis (CG) with spleen and stomach deficiency (SSD) and excess syndrome(ES) and relevant indexes. Methods: The distribution of TCM syndrome was observed in 69 cases of CG, and the relevant indexes of stomach dynamics were determined simultaneously. Multifactorial correlation analysis was done for TCM syndrome and indexes of functional test. Results: Most of CG in the early stage manifested as ES, and cases with deficiency syndrome (DS) occurred in older ages with longer course, suggesting that DS was more common in the middle or late stage. In ES group, the percentage of preprandial normal slow wave(Th) and dominant frequency (DF) as well as preprandial dominant power(DP) and postprandial DF showed a stronger positive correlation; the percentage of preprandial slow rhythm of stomach motion(B) and preprandial Th, and DF as well as preprandial B, postprantial B and postprandial Th showed negative correlation. In DS group, the gastric evacuation rate showed a statistical significane as compared with normal group, suggesting that the stomach evacuation rate was decreased in DS group following the increase of Tachygastria of preprandial gastric motion in digestive period, and there was a positive correlation between postprandial DP and preprandial DP. Conclusion: There is an unique objective basis in TCM syndromes which are independent of variation of regular pattern in CG.
