高龄结直肠癌外科治疗的探讨(附64例分析)

Surgical treatment of the elderly patients with colorectal carcinoma

目的 探讨高龄结直癌外科治疗。方法 总结我院 1986～ 2 0 0 1年 6 4例高龄结直癌外科治疗。结果 术前并存疾病的发生率为 5 3.1% ,2 9.7%同时合并两种以上的疾病 ,根治性切除率 6 0 .9% ,姑息性切除率 2 0 .3% ,肿瘤切除率 81.2 % ,术后并发症发生率 32 .8% ,围手术期死亡率 4 .7%。结论 手术切除是高龄结直肠癌病人最好的治疗方法 ,早期诊断 ,加强围手术期并存疾病和并发症的处理 。

Objective To explore the surgical therapy for the elderly patients more than 70 years old suffered from carcinoma of colorectal.Methods A total of 64 cases more than 70 years old were analyzed retrospectively from 1986 to 2001.Results Most of the patients had accompanied with other disease(53 1%),and 29 7% of the patients concurred with more than two kinds of diseases.The radical resection rate was 60 9%,palliative resection rate 20 3%,and total resection rate 81 2%,post operative complication rate was 32 8%,operation mortality rate 4 7%.Conclusion The operation is a best mechod to treat the elderly patients with colorectal carcinoma.Early diagnosis,correct management preexisting diseases and complication.appropriate choice of anesthesia and operation procedure.It is a key to inerease treatment result on the elderly patients with colorectal carcinoma.
