个体化颈椎弓根螺钉置钉技术的实验研究

Study of the Individual Cervical Pedicle Screws Placement Technique in Vitro

目的:探索一种较为安全、实用的颈椎弓根螺钉置钉方法。方法:随机取7具福尔马林浸泡的成人颈椎标本(C_1～T_2),保留完整的脊柱三柱结构及相关的韧带和椎旁肌,根据影像学测量的进钉点和进钉方向,模拟颈椎后路手术过程,进行实际的个体化置钉,摄正位、侧位、45°斜位X线片、CT扫描,进行解剖学观察,评价置钉的成败。结果:置钉84枚,失败7枚,失败率为8.3％。结论:个体化经颈椎弓根螺钉置钉技术比较安全和可靠,可以在临床上应用推广。

Objective: To explore a safe and effective method for placing the cervical pedicle screws. Methods:Seven adult cadaver specimens of cervical-thoracic spine (C1-T2) with intact structures including ligaments and perivertebral muscles were reserved and used. According to the radiographic measurements,an appropriate screw was inserted into pedicle individually. The evaluation of the screws' position was carried out by X-ray films in A-P,lateral and 45° oblique views,CT scans,and autopsy. Results:Eighty-four screws were placed,7 screws failed with a failure rate of 8. 3%. Conclusions:The individualized technique for placing cervical pedical screws is safe and effective,and can be applied and popularized clinically.
