甲亢病人TT_3水平与糖耐量和胰岛素释放功能关系

摘要 观察57例TT_3不同水平的单纯性甲亢及18例在治疗前后的单纯性甲亢病人的糖耐量—胰岛素释放功能。在TT_3正常值的22例中,其结果与对照组相符。在TT_3高于正常值的35例中,其结果与对照组有显著性差异,但OGTT仍在糖尿病曲线以下。18例在治疗前后的对比中发现:治疗前,由于TT_3增高,其糖耐量减低明显,而胰岛素分泌也明显升高;通过治疗,当TT_3降为正常值时,其OGTT及IRT也基本恢复正常。并就其机理进行了初步探讨。 Blood insulin release test (IRT)using the RTA method and oral glucose tolerance test (OGTT)was performed in 57 patients with hyperthyroidism. Twenty-two patients had a TT_3 value lower than 3.39 nmol/L. 35 patients had a TT_3 value higher than 3,54nmol/L. Among them, 18 patients had received the tests before and after treatment. Twenty normal persons were observed as control. The blood insulin levels using the OGTT were higher in cases of hyperthyroidism than that of normal persons. Eighteen cases with hyperthyroidism had a shortest glucose tolerance (33%). Changes of OGTT and IRT in hyperthyroidism were associated with elevation of TT_3 value. when the symptoms of hyperthyroidism were controlled, the TT_3 value reduced to normal, the OGTT and IRT became normal too. The results showed that the change of carbohydrate metabolism in most of patients with hyperthyroidism was due directly to the influonce of hyperthyroidism on carbohydrate metabolism in early stage, not due to the injury of pancreatic islet cells.
