脑卒中患者康复连续性管理模式初探

Exploration of the Continuous Management Mode on the Recovery of Stroke Patient

脑卒中是我国第1位的死亡原因。为提高脑卒中后患者的生存质量,降低脑卒中患者的复发率,上海市潍坊社区卫生服务中心与上海市仁济医院神经内科合作,通过开通就诊绿色通道及结合家庭医生工作成立管理团队的连续性规范化管理模式,降低了卒中患者的复发率、致残率,提高了卒中患者的生存质量。本文结合该社区的特点,对该连续性管理模式做一综述,分析该模式的优势,并对该模式未来的发展做一展望。

Stroke is the first cause of death in our country. To increase the quality of life and decrease the recurrence rate in stroke patients, our medical centre cooperated with Neurology Department of Shanghai Renji Hospital. Through launching green channel and making use of continuous management mode with family doctor team, we have decreased the recurrence rate and the disability rate, and increased the quality of life for them. This article reviewed the continuous management mode by combining with the feature of our community. It also analyzed the advantages and made a prospect on the mode.
