摘要
目的了解舟山市城市社区2型糖尿病(T2DM)患者的控制现状,为改进社区卫生服务相关工作提供依据。方法筛选工作基础较好、一般和比较薄弱的社区各1个,对在册管理1年以上,并且没有并发症的172名T2DM患者进行检查。结果 172名T2DM患者中,糖化血红蛋白<7.0%的为95名,控制率为55.23%,其中,男性为42.86%,低于女性的63.73%,差异有统计学意义(χ2=7.311,P<0.05)。空腹血糖为(4.4~7.0)mmol/L的有88名,控制率为51.16%,其中,男性为40.00%,低于女性的58.82%,差异有统计学意义(χ2=5.887,P<0.05)。其他指标中,高密度脂蛋白控制达标率最高,为88.37%,其次为血压、甘油三酯、总胆固醇和体质指数,达标率分别为62.21%、60.47%、52.91%和47.67%,控制达标率最低的是低密度脂蛋白,仅为43.60%。结论社区T2DM患者血糖控制现状还有改进空间,应加强糖尿病患者的个体化健康教育,增加患者的医学依从性,并干预其危险因素,改善患者的生活质量,延长健康生命年。
Objective To understand the status of blood glucose control in patients with type 2 diabetes mellitus(T2DM) in Zhoushan, to further improve the community health service related work. Methods Three communities were selected for this study based on their condition: good, fair, and weak respectively, and 172 T2 DM patients in these communities were given blood biochemical and physical examination for this study. Patients had to meet the following criteria, who have been registered for more than one year without complications. Results Of total 172 patients, 95 had Hb A1c<7.0%, the control rate was 55.23%, male 42.86%, female 63.73%, the gender difference was statistically significant(χ2=7.311, P<0.05); 88 patients had fasting blood glucose in the rage of(4.4-7.0)mmol/L, the control rate was 51.16%, male was 40.00%, lower than that of female of 58.82%, the difference was also statistically significant(χ2=5.887, P<0.05). Among other indicators, HDL-C controlling was the highest with a controlling rate of 88.37%, followed by BP, TC, TG and BMI, with the controlling rates were 62.21%, 60.47%, 52.91% and 47.67% respectively; the lowest rate was LDL-C, only 43.60%. Conclusion Blood glucose control status quo of these T2 DM patients indicated that their status could be improved. It is suggested to strengthen the individual health education, increase patient's medical compliance, manage their risk factors, improve their life quality, and prolong their health life year for diabetes patients.
