上海市北蔡社区外来儿童手足口病经济负担及影响因素调查

Analysis on Economic Burden and Influencing Factors Among the Migrant Child Patients with Hand-foot-and-mouth Disease at Beicai Town of Shanghai

目的了解上海市浦东新区北蔡社区外来儿童手足口病的经济负担及其影响因素。方法选取中国疾病预防控制信息系统中2015年7月—2016年6月北蔡社区报告的外来儿童手足口病病例,进行问卷调查,收集病例就医相关费用、就诊情况和医疗保险等信息并进行分析。结果共有203例病例纳入调查,其中,门诊病例199例,住院病例4例,无重症病例。门诊病例总费用中位数为558.00元,其中,直接医疗费用中位数为483.00元。交通费中位数为76.00元。单因素分析显示,是否发热和就诊次数是直接医疗费用和交通费的影响因素(P<0.01)。结论外来儿童手足口病给患者家庭造成了较重的经济负担。加强预防控制工作,降低发病率,及时治疗能有效减轻手足口病所带来的经济负担。

Objective To understand the economic burden and influencing factors among the migrant child patients with hand-foot-and-mouth disease(HFMD) at Beicai Town. Methods The data of migrant child HFMD cases at Beicai Town which were reported in the National Disease Surveillance Report Management System from July 2015 to June 2016 were collected to analyze the expenditure of clinical visiting, hospital visit status, and medical insurance, etc. Results A total of 203 cases, which included 199 outpatient cases and 4 inpatient cases, were investigated. The median of total cost was 558.00 yuan. The median of the direct medical cost and the transportation cost per HFMD outpatient case were 483.00 yuan and 76.00 yuan respectively. The results of single factor variance analysis showed that the times of clinical visiting and whether they had fever were the influencing factors of direct medical cost and the transportation cost(P<0.05). Conclusion HFMD causes heavier economic burden on the migrant child patients' families. Strengthening the prevention and control of HFMD, reducing the incidence of HFMD, and timely treatment of HFMD would lighten the economic burden of HFMD.
