超声心动图联合心电图对心尖肥厚型心肌病诊断应用价值

Diagnostic value of echocardiography and electrocardiogram in apical hypertrophic cardiomyopathy

目的探讨超声心动图结合心电图诊断心尖肥厚型心肌病的临床价值。方法选取北部战区总医院自2018年1月至2019年3月收治的60例心尖肥厚型心肌病患者为观察组,另选取60例同期健康志愿者作为健康组。两组均接受超声心动图及心电图检查,并对检查结果进行分析比较。结果与健康组比较,观察组左心房收缩末期内径、左心室心尖部最大厚度、左心室侧壁中下段最大厚度均增加,E/A、二尖瓣环及侧壁e均降低,差异均有统计学意义(P<0.05)。观察组患者心尖部心肌显著增厚,心电图出现左心室高电压与左胸前导联ST段下移、巨大倒置T波。结论超声心动图结合心电图对准确诊断心尖肥厚型心肌病具有重要价值。

Objective To investigate the clinical value of echocardiography combined with electrocardiogram in the diagnosis of apical hypertrophic cardiomyopathy(ApHCM)and to evaluate the diastolic function of myocardium.Methods A retrospective study was performed on 60 cases of patients with ApHCM(observation group)who were admitted from January 2018 to March 2019.Another 60 healthy volunteers in the same period were selected as the healthy group.Echocardiography and electrocardiogram were performed in both groups,and the results were analyzed.Results Compared with healthy group,the left ventricular end systolic diameter and left ventricular apex of maximum thickness,left ventricular wall thickness of the middle period of the largest increased,E/A,mitral valve ring and side wall E were reduced in the observation group,and the differences were statistically significant(P<0.05).In the observation group,the apical myocardium was significantly thickened,and the electrocardiogram showed the left ventricular high voltage and the leading ST segment down and the huge inverted T wave on the left chest.Conclusion Echocardiography combined with electrocardiogram is of great value in the accurate diagnosis of ApHCM.
