摘要 目的探讨急性油漆中毒对海勤人员生化指标和肺通气功能的影响,为合理制定舰艇舱室刷漆作业的劳防措施提供依据。方法选择2010年10月-2013年10月某院收治的急性油漆中毒海勤人员9例作为调查组,选择同期健康陆勤人员10例作为对照组。观察急性油漆中毒临床症状、生化指标异常及治疗情况,分析调查组痊愈后肺通气功能情况。结果 9例均有头昏和乏力,生化指标异常主要表现为丙氨酸转氨酶、乳酸脱氢酶、总胆红素、肌酸激酶升高,血钾、纤维蛋白原、动脉血氧分压降低,经对症治疗后临床痊愈。调查组临床痊愈后肺通气功能指标均恢复至正常值范围,但仍低于对照组,两组比较差异无统计学意义(P>0.05)。结论舰艇舱室刷漆作业有油漆二甲苯中毒危险,发生急性中毒程度较轻、肺通气功能损害可逆。 Objective To explore the changes of pulmonary ventilation functions of shipboard naval personnel with acute oil-based paint poisoning and provide guidelines for labor protection of paint work in the naval vessel. Methods Nine shipboard naval personnel with acute paint poisoning hospitalized between October 2010 and October 2013 were chosen as the study group while ten healthy military personel as the control group. The clinical symptoms,biochemical indexes and effect of treatment were observed,and pulmonary ventilation functions were analyzed. Results All the nine patients in study group felt dizzy and weak,and their levels of alanine aminotransferase,lactate dehydrogenase,total bilirubin,creatine kinase all increased while their blood potassium,fibrinogen,arterial blood oxygen partial pressure decreased. All the patients were cured after treatment. Pulmonary ventilation function indexes returned to normal after recovery,but still lower than those in control group. The difference,however,was not statistically significant( P>0.05). Conclusion When exposed to xylene in oil-based paint in naval vessels,shipboard naval personnel is vulnerable to acute poisoning. Although the poison does not cause significant or irreversible damage to pulmonary ventilation functions,labor protection is indispensable.
