摘要
目的分析对进行通气治疗的慢阻肺急性加重患者实施肠内免疫营养支持的临床效果。方法选择我院自2014年1月1日-2017年10月1日收治的160例慢阻肺急性加重患者作为研究对象,所有患者均行机械通气治疗,随机将患者分成对照组和观察组,每组80例,对照组施行常规肠内营养支持治疗,观察组施行肠内免疫营养支持治疗,比较两组患者治疗前后的机体营养指标(包括转铁蛋白、白蛋白和血清前白蛋白水平)、机械通气治疗时间、住院时间、治疗2周后的脱机成功率及患者通气治疗期间的不良反应发生率。结果两组患者治疗前的各项营养指标对比无显著差异(P>0.05),治疗后,观察组患者的转铁蛋白、白蛋白和血清前白蛋白水平、治疗2周后的脱机成功率均明显高于对照组,观察组患者的机械通气治疗时间、住院时间及通气治疗期间的不良反应发生率明显低于对照组,两组对比差异显著(P<0.05)。结论行通气治疗的慢阻肺急性加重患者实施肠内免疫营养支持治疗,不仅能改善患者的机体营养状况,同时还能有效缩短患者通气治疗周期并降低患者通气治疗期间的不良反应发生率,是一种高效的辅助治疗方案。
Objective To analyze the clinical effect of enteral immunonutrition support in patients with acute exacerbation of chronic obstructive pulmonary disease treated with ventilation.Methods A total of 160 patients with acute exacerbation of chronic obstructive pulmonary disease admitted to our hospital from January 1,2014 to October 1,2017 were enrolled.All patients underwent mechanical ventilation and were divided into controls by mean grouping.Group and observation group,80 cases in each group,the control group received routine enteral nutrition support treatment,the observation group received enteral immunonutrition support treatment,compare the body nutrition index before and after treatment(including transferrin,albumin and serum)Pre-albumin levels),mechanical ventilation treatment time,length of hospital stay,offline success rate after 2 weeks of treatment,and incidence of adverse reactions during patient ventilation.Results There was no significant difference in the nutritional indexes between the two groups before treatment(P>0.05).After treatment,the transferrin,albumin and serum prealbumin levels in the observation group were offline after 2 weeks of treatment.The rate was significantly higher than that of the control group.The incidence of adverse reactions in the observation group,hospitalization time and ventilation treatment was significantly lower than that in the control group.The difference between the two groups was significant(P<0.05).Conclusion Intestinal immunonutrition support therapy for patients with acute obstructive pulmonary disease exacerbation with ventilation therapy can not only improve the patient's nutritional status,but also effectively shorten the patient's ventilation treatment cycle and reduce the incidence of adverse reactions during ventilation therapy.It is therefore an efficient adjuvant treatment option.
