摘要 目的探究血清维生素A和维生素E水平与小儿急性下呼吸道感染(ALRI)的相关性。方法分别选取我院儿科2016年10月-2018年10月收治的ALRI患儿100例和体检中心的健康儿童100例。采用问卷调查方法收集所有受试者性别、年龄、是否早产儿、母乳喂养、家庭吸烟接触等信息。采用高效液相色谱法测定血清维生素A和维生素E水平。采用无条件Logistic回归分析不同因素与小儿ALRI风险的相关性。结果两组受试者性别、年龄和家庭吸烟接触差异无统计学意义(P>0.05)。然而,病例组中的早产儿显著多于对照组,母乳喂养少于对照组,差异具有统计学意义(P<0.05)。病例组的血清维生素A和维生素E水平显著低于对照组,差异具有统计学意义(P<0.05)。此外,将病例组按照不同疾病类型对血清维生素A和维生素E水平进行比较,可见ALRI患儿随着疾病严重程度加重,血清维生素A和维生素E水平越低,差异具有统计学意义(P<0.05)。多因素分析结果表明,维生素A<0.3 mg/L、维生素E<7 mg/L均为小儿ALRI的独立风险因素(P<0.05)。结论血清维生素A和维生素E偏低均为小儿ALRI的独立风险因素。 Objective To investigate the association between serum vitamin A and vitamin E levels and acute lower respiratory tract infection(ALRI)in children.Methods A total of 100 children with ALRI and 100 healthy childrenwere recruited from October 2016 to October 2018 in the department of pediatrics and physical examination center of our hospital,respectively.Questionnaires were used to collect the sex,age,premature birth,breastfeeding,and family smoking exposure of all subjects.Serum vitamin A and vitamin E levels were determined by high performance liquid chromatography.Unconditional logistic regression was used to analyze the correlation between different factors and the risk of ALRI in children.Results There was no significant difference in gender,age and family smoking exposure between the two groups(P>0.05).However,the preterm infants in the case group were significantly more than the control group,and breastfeeding was less than the control group,the difference was statistically significant(P<0.05).Serum vitamin A and vitamin E levels in the case group were significantly lower than those in the control group,and the difference was statistically significant(P<0.05).In addition,comparing the serum vitamin A and vitamin E levels in the case group according to different disease types,it can be seen that the serum vitamin A and vitamin E levels were gradually decreased with the severity of the disease in children with ALRI,the difference is statistically significant(P<0.05).Multivariate analysis showed that infants,vitamin A level lower than 0.3 mg/L,and vitamin E level lower than 7 mg/L were independent risk factors for ALRI in children(P<0.05).Conclusion Low serum vitamin A and vitamin E are independent risk factors for pediatric ALRI.
