摘要
目的研究白细胞VCS参数在白血病诊断中的临床应用。方法用Coulter LH750血细胞分析仪对112例白血病患者和50例正常对照组进行白细胞分类检测,记录中性粒细胞和淋巴细胞的V、C、S测定值,用SPSS13.0做统计学分析。结果白血病患者中性粒细胞平均体积、中性粒细胞体积分布宽度、淋巴细胞平均体积、淋巴细胞体积分布宽度、淋巴细胞平均激光率的结果明显高于对照组,分别是144.8±10.3VS141.5±3.7、22.4±7.9VS18.5±0.9、87.8±7.4VS83.9±2.0、14.8±3.1VS13.2±0.8、66.2±10.3VS61.8±2.1,差别比较有显著意义(P【0.05)。白血病患者中性粒细胞平均传导率、淋巴细胞平均传导率的结果低于对照组(139.8±4.3VS142.0±1.8、106.9±3.6VS108.2±1.2),差别比较有显著意义(P【0.05)。结论白血病患者的外周血白细胞形态和结构变化可以通过VCS参数定量地测定,部分参数如中性粒细胞体积分布宽度、淋巴细胞平均体积是白血病筛查、诊断的新指标。
Objective To investigate the clinical application of VCS parameters of leukocyte in the diagnosis of leukemia.Methods Peripheral blood samples were drawn from 112 leukemia patients and 50 normal donors, and leukocyte cells were measured with Coulter LH750 hematology analyzer. The values of volume, conductivity, scatter of neutrophil and lymphocyte were measured and recorded. SPSS13.0 was used for statistics analysis.Results In leukemia patients,the mean channel of neutrophil volume(MNV)、neutrophil volume distribution width(NDW)、mean channel of lymphocyte volume(MLV)、lymphocyte volume distribution width(LDW) and mean channel of lymphocyte scatter(MLS) were significantly higher compared to normal donors (144.8±10.3 VS 141.5±3.7、22.4±7.9 VS 18.5±0.9、87.8±7.4 VS 83.9±2.0、14.8±3.1 VS 13.2±0.8、66.2±10.3 VS 61.8±2.1, respectively),P<0.05. However, mean channel of neutrophil conductivity (MNC) and mean channel of lymphocyte conductivity (MLC) were significantly lower in leukemia patients compared to the normal donor (139.8±4.3 VS 142.0±1.8、106.9±3.6 VS 108.2±1.2, respectively), P<0.05.Conclusion The morphologic changes of leukocyte in peripheral blood in leukemia patients can be quantitatively measured by the VCS parameters. Some parameters such as NDW、MLV are new indicators for the screen and diagnosis of leukemia.
