脊柱显微内镜治疗腰椎间盘突出症手术原则

EXPLORATION OF OPERATIVE DOCTRINE OF MICROENDOSCOPIC DISECTOMY

目的 :探讨后路脊柱显微内窥镜椎间盘摘除术 (MicroendoscopicDisectomy ,MED)治疗腰椎间盘突出症(LumbarDiscHemiation ,LDH)手术的原则。方法 :回顾总结 348例腰椎间盘突出症行MED手术的临床资料。结果 :本组 336例获得随访 ,随访时间平均 1 8个月 ,按照Macnnab标准评定 ,优 2 69例 ,良 38例 ,可 2 9例 ,优良率 91 .2 5 %。结论 :MED手术成功的关键是必须遵守MED手术原则 ,即 :正确的手术适应证、熟练的操作技术。

Objective:To explore the operative doctrine of lumbar disc herniation (LDH) with microendoscopic disectomy (MED) through posterior approach. Method:Retrospective analyses the clinical data of 348 cases of lumbar disc herniation with MED.Result:Three hundred and thirty-six cases were followed-up for average 18 months. According to the Macnnab scale, 282 cases were evaluated as excellent, 38 cases as good, 16 cases as fair. Conclusion: It is essential for successful operation to abide by common doctrine of MED. That is accurate surgical indication,having skilled operative technique of MED, correct disposal of intraoperative accident.
