涎腺腺样囊性癌的治疗体会

Treatment of Adenoid Cystic Carcinoma of Salivary Gland

作者根据临床经验和文献复习 ,对涎腺腺样囊性癌的治疗提出以下体会。认为①应当废除那种把腺样囊性癌当作低度恶性处理的老观念 ,应有足够手术切除 ;②应当根据不同的原发部位特点 ,制订不同手术原则 ;③术后常规放射治疗是需要的 ;④术前严禁作切开活检 ,采用术前细针穿活检或术中冰冻切片病理检查 ;适当病例可采用术后化疗。

This paper introdueed the treatment experience on adenoid cystic carcinoma of the salivary gland based on the author's clinical practice and literature review.It is concluded that:①Enough safe surgical margins should be guaranteed and the old concept of treating adenoid cystic carcinioma as low grade malignant tumor should be abolished;②The treatment plan should be designed according to the different primary sites;③Postoperative radiation should be applied routinely in all patients;④Incisional biopsy should not be performed prior to operation.Fine needle aspiration cytology and intraoperative frozen pathology are advocated.Postoperative chemotherapy can be used in selected cases.
