胃癌患者血清血管内皮生长因子含量分析

The Determination of Serum Vascular Endothelial Growth Factor in Gastric Cancer

[目的]测定胃癌患者血清血管内皮生长因子 (VEGF)的含量 ,分析其与胃癌临床病理指标之间的关系。[方法]采用ELISA法测定60例胃癌患者血清VEGF含量 ,正常对照组20例。[结果]胃癌患者初治组及术后复发转移组治疗前血清VEGF浓度分别为(275.72±223.28)pg/ml和(304.47±157.19)pg/ml均明显高于正常对照组浓度(105.82±23.67)pg/ml(P<0.001,P=0.003)。胃癌淋巴结阳性组患者血清VEGF明显高于阴性组 (340.09±244.47)pg/mlvs(138.93±44.89)pg/ml(P<0.001) ,远处转移组明显高于未转移组,(501.16±290.62)pg/mlvs(179.10±70.98)pg/ml(P<0.001) ,并随着浸润深度及TNM分期的增加 ,其浓度也随之增高 (P=0.002,P<0.001) ,但Ⅰ期患者与正常对照组无差异 (P=0.276)。血清VEGF浓度与性别、年龄、发病部位、分化程度无相关性 (P>0.05)。[结论]VEGF与胃癌的生长、浸润、转移等生物学行为有关 ,可能是判断预后的一个血清学指标。作为早期诊断胃癌的指标其应用价值不大。测定术前VEGF浓度可以预测临床分期、淋巴结及远处转移情况 ,对指导临床选择恰当的综合治疗手段有一定的临床意义。

Purpose To determine the correlation between pretreatment serum vascular endothelial growth factor (VEGF) and clinicopathologic features in patients with gastric carcinoma.Pretreatment serum VEGF was measured by enzyme-linked immunosorbent assay(ELISA) in 20 healthy controls and 60 patients with gastric carcinoma.Serum VEGF was found to be significantly elevated in patients with primary gastric carcinoma (P<0.001) and with recurrent tumors (P=0.003) in contrast to controls. Significant differences were observed when serum VEGF was categorized by tumor depth(P=0.002),lymph node metastasis (P<0.001),distant metastasis (P<0.001 )and TNM stage (P<0.001), but no differences were observed between the serum VEGF of stageⅠand that of normal control(P=0.276). There were no significant association among gender,age,location,histology grade with serum VEGF(P>0.05).[Conclusion]Serum VEGF was associated with the development,invasion,metastasis and other biological behaviors of gastric carcinoma.It is not a useful marker for diagnosis of early gastric carcinoma.It has some significance for choice of multimodality therapy.
