喉癌术后并发咽瘘相关因素的探讨

Study of the Predisposing Factors of Pharyngo - cutaneous Fistulae Following Laryngeal Cancer Surgey

目的 探讨喉癌术后咽瘘发生的相关因素。方法 对本院1993～2001年喉癌手术患者的临床资料进行回顾性分析,应用SAS软件进行统计学分析。结果 98例患者中13例发生术后咽瘘(13.26％),其发生与肿瘤临床分期、手术方法、术前放疗、手术时间和术后感染有关。结论 喉癌的早期诊断、治疗对于预防术后咽瘘的发生至关重要,而手术者技术的熟练、缩短手术时间、术后感染的及时预防和正确处理,有助于预防咽瘘的发生。

Objective To search the predisposing factors of pharyngo - cutaneous fistulae after surgical treatment oi laryngeal cancer. Methods Ninety - eight hospitalized patients who underwent surgical treatment from 1993 to 2001 were retrospectively studied. Results The incidence of post - operative pharyngo -cutaneous fistulae was 13.26% (13/98). Their prevalence depended on clinical tumor stage, surgical approach, preoperative radiotherapy, operative duration and post - operative infection. Conclusion Early diagnosis and prompt treatment are crucial to prevent this complication; skillful operative technique, shortening of operative duration and timely and effective prevention of post - operative infection may be contributed to reduce the incidence of pharyngo - cutaneous fistulae after surgical treatment of laryngeal cancer.
