短节段Silhouette内固定结合BAK治疗腰椎滑脱 被引量:1

摘要 目的 观察腰椎管减压、多孔螺纹状钛合金椎间融合器 (BAK)、椎体间植骨、短节段Silhou ette内固定治疗腰椎滑脱的早期临床疗效。方法 采用腰椎管减压、BAK、椎体间植骨及短节段Sil houette内固定手术治疗 18例腰椎滑脱患者。术前经X线检查后按Meyerding分度 :Ⅰ度滑脱 15例 ,Ⅱ度滑脱 3例 ;滑脱部位 :L410例 ,L58例。结果 术后平均随访 7.4个月 (3~ 18个月 ) ,18例患者中 ,17例症状术后完全消失 ,优良率达 94 .4 %。 18例滑脱椎体均理想复位。无一例发生感染 ,1例因合并有颈椎管狭窄而残留轻度行走不稳。结论 运用短节段Silhouette内固定结合BAK治疗腰椎滑脱复位满意 ,效果优良。 Objective To observe the early clinical outcome of lumbar spondylolisthesis treated with decompressive laminetomy, intertransverse corpus vertebrae arthrodesis with bone grafting, Ti alloy porous threaded fusion cage(BAK) and transpedicle instrument of short segmental Silhouette system. Methods 18 cases of lumbar spondylolisthesis were treated with decompression, intertransverse corpus vertebrae arthrodesis with bone grafting, BAK interbody fusion cage and transpedicle instrument of short segmental Silhouette system. X ray pictures were taken before operation. According to Meyerding classification system of lumbar spondylolisthesis, 15 cases were classified as degree 1, 3 cases degree 2. The levels of lumbar spondylolisthesis were L 4 in 1 0 patients and L 5 in 8 cases. Results The patients were followed up from 3 to 18 months, with an average of 7.4 months. The results showed that the pre operative clinical symptoms disappeared completely in 17 of 18 patients, pain relief in 94.4%. All cases obtained complete reduction of the olisthy vertebral bodies. No infection was found in this series. One case associated with cervical stenosis, and the claudication was not recovered completely. Conclusion It has satisfactory reduction rate and excellent clinical results to treat lumbar spondylolisthesis with BAK interbody fusion cage and short segmental Silhouette system.
