帕金森病患者抑郁的相关因素分析

Analysis of Depression in Patients with Parkinson’ Disease

目的 了解帕金森病患者抑郁的发生率 ,分析其影响因素和可能的病因。方法 以改良的Weber评分法、Hoehn -Yahr分级评定患者的病情严重程度 ,UPDRⅢ评定患者的运动功能 ,Hamilton抑郁量表 (2 4项 )评定患者的抑郁程度 ,日常生活能力量表评定患者的生活能力 ,并计算患者服用左旋多巴的总剂量和平均每日剂量。结果 45例帕金森患者中 ,有 2 0例发生抑郁 ,发生率为 44 .4%。经回归分析发现抑郁程度与H -Y分级呈正相关 ;与年龄、UPDRSⅢ评分、平均每日左旋多巴剂量呈负相关。偏相关分析发现 ,平均每日左多巴剂量其偏相关系数为- 0 .3 5 12 (P =0 .2 90 ) ,无统计学意义。结论 抑郁症在帕金森病患者中较为普遍 。

Objective To find the occurrence of depression in Parkinson' disease and the factors related to it. Method Disease severity was evaluated by Hoehn-Yahr stages and improved Weber scores.The level of depression was evaluated by Hamilton rating scales (24 items).The capability of life was evaluated by ADL(activity of daily life).The movement functions was evaluated by UPDRSⅢ.Both the daily dosage and the gross dosage of L-dopa were calculaled. Results Twenty of 45 ptients with Parkinson' disease have depression and the occurrence rate was 44.4%, Using stepwise liner regression and partial correlation, we have found that the level of depression was associated with the severity of Parkinson' disease, the on-period scores of UPDRSⅢ,and the age. Conclusion Depression is very common in Parkinson' disease and may be resulting from pathological changes of brain caused by Parkinson' disease.
