米力农与FDP治疗难治性心力衰竭的效果

EFFICACY AND SAFETY OF MILRINONE AND FRUCTOSINI NATRIL DIPHOSPAS IN THE TREATMENT OF REFRACTORY HEART FAILURE

①目的 探讨难治性心力衰竭 (RHF)的有效治疗方法。②方法 按NYHA心功能分级标准 ,选择心功能Ⅳ级均经利尿剂、血管紧张素转换酶抑制剂、洋地黄制剂、血管扩张剂综合治疗无效的RHF病人 40例 ,加用米力农与FDP静滴 1周 ,观察治疗前后心功能的变化。③结果 治疗后近期显效率为 2 5 .0 % ,有效率为 67.5 % ,总有效率为 92 .5 %。治疗后左室收缩与舒张功能障碍明显改善 (t=6.3 14、15 .812 ,P <0 .0 1) ;心室率减慢 (t=2 .2 3 3 ,P <0 .0 5 )。 1例因静滴速度快出现一过性低血压 ,轻微头晕、恶心。④结论 米力农与FDP联合静脉点滴治疗RHF近期疗效满意 。

Objective To study an effective therapy for refractory heart failure (RHF). Methods Forty patients with RHF whose heart function was classified as grade Ⅳ , according to NYHA cardiac function criteria, received diuretic and nitroglycein without effect at first, and then milrinone and fructosini natril diphosaps were added for one week. The changes of the heart function before and after using milrinone and fructosini natril diphosaps were compared. Results The short-term marked effective rate was 25.0%, the effective rate was 67.5% and the total effective rate was 92.5%. The systolic and diastolic function of the heart improved greatly(t=6.314,15.812,P<0.01). The heart rate lowered (t=2.233,P<0.05). Transient low blood pressure, slight dizziness and nausea occurred in one case due to rapid intravenous dripping. Conclusion The short-term therapeutic effect is satisfactory by combination of milrinone and fructosini natril diphospas for refractory heart failure. It can improve systolic and diastolic function of left ventricle.
