螺旋CT在胰腺癌诊断中的价值及临床应用

Dual-phase spiral CT:value for diagnosis of pancreatic carcionma and clinical application

目的 :探讨螺旋CT在胰腺癌中的诊断价值及临床应用。方法 :回顾性分析了 48例胰腺癌患者螺旋CT图像 ,扫描时间为注射对比剂后 40～ 55s和 60s左右 ,并依照正常胰腺组织和肿瘤在平扫和两期的增强情况、CT征象行术前评估 ,再与手术结果对照。结果 :胰腺 -肿瘤增强密度差异值平均分别为胰腺期 49.1 0HU、门静脉期 2 7.0HU ,两期差异有显著意义 (t=2 .543、P <0 .0 1 )。肿块在胰腺期能够清晰显示。 2 5例术前认为可切除的肿瘤 ,术中仍有 7例不能切除。结论

Objective:To study the evaluation of dual phase CT in the diagnosis of pancreatic carcinoma and clinical application.Methods:Retrospectively analyze spiral CT was performed on 48 patients with pancreatic carcinoma after intravenous contrast material injected 40～55 seconds and 60 seconds.Compare the enhancement of normal pancreas and tumor.Diagnose pancreatic carcinoma and assess of tumor resectability based on CT findings was compared with surgical results.Results:Density differenc of pancreas to tumor contrast was average of 49.10HU in the pancreatic phase and 27.0HU in the portal phase,the difference of the two phases was greatest(t=2 543、P<0.01),tumors were visualized well in the pancreatic phase.25 tumors were thought of resectable before operation,but 7 tumors were not resectable during operation.Conclusion:Dual phase spiral CT can enhance the rate of accuracy in diagnosis of pancreatic carcinoma.It had an important value in assessment tumor resectability before the operation.
