肥胖相关性肾病的超微结构观察

An ultrastructural study on obesity-related glomerulopathy

目的 :观察肥胖相关性肾病 (ORG)的超微结构改变特点。 方法 :选择体重指数 (BMI) >2 8kg/m2 、肾活检光镜改变符合ORG的患者 6例作为观察对象 ,透射电镜下观察其肾活检组织的超微结构改变特点 ,并随机选择特发性局灶节段性肾小球硬化 (I FSGS)患者 6例作对照。 结果 :发现ORG组肾小球脏层上皮细胞肿胀 ,内见蛋白吸收滴及脂质沉积 ,与I FSGS组相比 ,其广泛的足突融合较少 (35 %vs 6 0 % ,P <0 .0 5 ) ,足突微绒毛样改变也较少(30 %vs 6 5 % ,P <0 .0 5 )。 结论 :ORG组肾小球上皮细胞足突融合少 。

Objectives:To observe the characters of ultrastructural changes of obesity related glomerulopathy (ORG) by transmission electron microscope. Methods: Six obese patients(body mass index>28 kg/m 2 ) with biopsy proven ORG by light microscope were compared with a control group of six non obese patients with idiopathic focal segmental glomerulosclerosis(I FSGS). Results: We found mild podocyte hypertrophy and intracytoplasmic protein resorption droplets and lipid droplets. Comparing with control group I FSGS, there were significant less foot process fusion (35% vs 60%, P <0.05) and like microvillous of foot process changes(30% vs 65%, P <0.05). Conclusions:There were significant differents in ultrastructure between the groups of ORG and I FSGS.
