角膜切口小梁切除术临床分析

Clinical analysis of trabeculectomy with corneal incision

目的 探讨一种不易引起滤过区瘢痕组织形成、疗效稳定的治疗青光眼的手术方式。方法 对传统的小梁切除术的切口及巩膜瓣进行改良 ,采用角膜切口、不作球结膜瓣 ,巩膜瓣作成隧道式、于隧道底切穿浅层巩膜形成滤过通道。结果 术中术后并发症少 ;远期疗效好 ,经长期随访眼压正常 ,滤过分布广泛 ,滤过泡无缩窄或消失 ;大部分视力不同程度提高。结论 角膜切口小梁切除术是一个并发症少 ,成功率高 。

Objective To investigate operative method to cure glaucoma which is not easy to lead to filtrationzone scar and has stable treatment effect.Methods The incision and sclera valve of traditional trabeculectomy were improved in this operative method,in which corneal incision was adopted and sclera valve rather than valve of bulbar conjunctiva was made.The sclera valve was made a tunnel,in the bottom of which superficial layer of sclera was cut to form filtration passage.Results The operative method had less complication and better treatment effect.By long term following up,normal ocular tension,extensive filtration distribution and filter follicle without stenosis of disappearance were found.To different degree,the eyesight obtained promotion in most patients.Conclusions The trabeculectomy with corneal incision is a ideal operative method to cure glaucoma,which has less complication,high success rate and stable treatment effect.
