老年无牙颌患者的修复治疗

Restoration treatment of elderly patients with edentulous

目的:总结老年无牙颌患者全口义齿修复的临床体会。方法:对66例老年无牙颌患者采用二次印模法取印模,制作蜡堤,确定垂直距离,取正中颌关系,经排牙、烫蜡、装盒、装胶、热处理、打磨等工序完成义齿制作,义齿初戴、调磨,戴用后随访,记录患者复诊情况。结果:59例患者戴用义齿后,经1~3次复诊后,固位良好,咬合关系良好,患者表现为满意。5例患者复诊3次以上,经复诊调改后义齿固位改善,不适缓解,咀嚼效率提高,患者基本满意。2例患者不满意,均返工重做。结论:全口义齿制作步骤繁琐,在修复过程中要做到严格、细致、准确,同时注重临床经验的积累及总结。对于老年患者需考虑身体素质、适应情况等具体因素,积极交流沟通,以取得患者的配合,获得最佳修复效果。

Objective:To summarize clinical experience of complete dentures restoration.Methods:66cases of elderly edentulous patients were made wax embankment by secondary impression method,and determined thejaw relation.The production of dentures was completed after tooth arrangement,waxing,packing and other processes.New dentures were grinded.Follow-up visits were recorded after new dentures usage.Results:59patients after further consultation showed satisfactory.5patients showed basic satisfactory after more than three times further consultation.2patients were not satisfied with their dentures,and all artificial teeth were reworked.Conclusions:The production steps of complete dentures are complicated.Every step should be strict,careful,and accurate.Clinical experience should be accumulated and summarized.Elderly patients need to be considered their physical quality,and their adaption to specific circumstances.Positive communication is very necessary in order to obtain patient's cooperation for the best restoration.
