周边视网膜切除治疗伴aPVR的视网膜脱离

Peripheral Retinectomy for the Treatment Severe Anterior Proliferaflve Vitroretinopathy

目的观察周边部视网膜切除治疗伴有重度前部增殖性玻璃体视网膜病变(anterior proliferativevitroretionpathy,aPVR)的视网膜脱离的疗效并探讨其并发症及预防的方法。方法应用玻璃体视网膜手术结合治疗17例伴有重度aPVR的视网膜脱离,术中都采取了不同范围的周边部视网膜切除。结果随访3～18个月,平均12.3个月,术后视力在0.01以上9例,占52.9%;视网膜复位者11例,占64.7%。结论尽管传统手术治疗伴有重度aPVR的视网膜脱离的效果不理想,但是经过这种手术方式的治疗能使多数患者保持和改善部分的视力。

Purpose To investigate the effect of peripheral retinectomy for the treatment of retinal detachment associated with severe anterior proliferative virroretinopathy, and to discuss its complication and preventive management. Methods 17 cases of retinal detachment associated with severe anterior proliferative vitroretropathy were treated with vitrous retina surgery. During the surgery, different coverage of peripherial retinectomy were applited. Results The visual acuity in 9 cases was 0.01 or better (52.9%). The retinas in 11 (64.7%) cases were reattached 3 to 18 months after operations (average 12.3 months). Conclusion Although the effect of traditional treatment for retinal detachment associated with severe aPVR is not satisfied, but by using this kind of surgery, most of them can maintain or improve their visual acuity.
