超声评价高血压患者外周动脉粥样硬化

Ultrasonic assessment of peripheral atherosclerotic lesions in hypertension patients

目的 探讨高频超声在检测高血压患者外周动脉粥样硬化中的临床应用价值。方法 60例高血压患者分无并发症及有并发症两组,分别用高频超声评价颈总动脉(CCA)内径、内膜-中层复合体厚度(IMT)、动脉粥样硬化发生比例、回声特点及肱动脉内皮舒张功能。结果 高血压两组IMT、动脉粥样硬化发生比例均高于对照组,其中动脉粥样硬化发生率高血压有并发症组明显高于无并发症组;高血压两组内皮依赖性舒张功能较对照组明显下降,而非内皮依赖性舒张功能3组间无明显差异。结论 超声检测CCA内径、IMT及肱动脉内皮依赖性舒张功能对高血压外周动脉粥样硬化有早期诊断作用,IMT增厚及肱动脉内皮依赖性舒张功能下降可作为评价高血压外周血管病变的一项指标。

Objective To study the diagnostic value of high-frequency ultrasound of peripheral atherosclerotic lesion in hypertension patients. Methods Sixty subjects, divided into hypertension group and without-complication group, were examined by sonography. The common carotid artery (CCA) diameter, intima-media thickness(IMT),atherosclerotic existence rate,echo characteristics and endothelial dilation function of the brachial artery. Results IMT and atherosclerotic existence rates of the hypertension group and without-complication group were higher than those in normal subjects. IMT and atherosclerotic existence rate in hypertension group was significantly higher than that of without-complication group. Endothelial dependent vasodilation function of hypertension group and without-complication group was decreased significantly than those in normal group, but endothelial-in-dependent vasodilation function did not differ significantly among the three groups. Conclusions The carotid IMT and endothelial dependent vasodilation function in brachial arteries have the early diagnostic value with peripheral arteriosclerosis of hypertension patients and can be used as a kind of index in evaluation of the peripheral vascular disease of hypertension.
