摘要
目的:探讨胸骨骨折钢丝内固定的最佳手段。方法:将骨折之胸骨处向上,下两端分别游离约3.5cm,在3.0cm处用8号钛合金钢丝横绕胸骨,扭紧钢丝,在钢丝远端分别等距钻两个孔,铜丝穿过孔眼纵行固定。结果:本组11例,1例死亡,1例因剧烈咳嗽引起钢丝松动,在局麻下扭紧钢丝,余无钢丝断裂及胸骨撕裂者,临床效果满意。结论:“井”字形钢丝内固定治疗创伤性胸骨骨折,因纵行钢丝着力点在横行钢丝上,减少了纵行钢丝在局部胸骨上的切割力,避免了胸骨撕裂伤,降低了因胸骨撕裂而重复手术的机会,使病人能早愈合早出院减轻痛苦,降低费用。
Objective: To explore the best means of using steel wire to interior fixing breastbone fracture. Methods: We separated the two sides of breastbone above and down the fracture about 3.5 centimeter. At about 3.0 centimeter above and down the fracture we used 0.8 millimeter titanium alloy steel wire horizontal wind the breastbone, and turn round the wire till tight. Then at the distant sides of steel wires, we drilled two holes distinguish and let the wires through out the holes then longitudinal fixed. Results: There were 11 cases in this group. One of these patients dead. One lead to the steel wires flexible because of acute cough. We turned round the steel wires tight. There was not steel wires break apart and no breastbones was treated. The clinical effects were pleased. Conclusions: Because the spots of accept force of the longitudinal steel wires were on the horizontal steel wires, the cut force of longitudinal steel wires on the parts of breastbone be cut down, the breastbone being tear were avoided, and the chances of repeated operations were cut down because of this. This make patients recover earlier and out of hospital earlier. This could reduce the pain of them and drop the fee.
