抗细胞间黏附分子-1单抗对心肌缺血再灌注早期和远期心功能的影响

Effect of anti-ICAM-1 monoclonal antibody on cardiac function in rats with early or long term reperfusion after myocardial ischemia

摘要 目的 研究抗细胞间黏附分子 1(ICAM 1)单抗对大鼠心肌缺血再灌注早期和远期死亡率及心功能的影响。方法 应用大鼠心肌缺血再灌注 2 4h和再灌注 6周模型。以氯化 2 ,3,5 三苯基四氮唑 (TTC)染色法确定心肌坏死范围。血流动力学分析系统测定平均动脉压、左心室收缩期压力峰值 (LVSP)、左心室舒张末期压 (LVEDP)、左心室最大上升速度 (+Pmax)和下降速度 (-Pmax)等指标。结果 用药组大鼠缺血 4 5min再灌注 2 4h ,与对照组相比死亡率差异无显著性意义 (P >0 .0 5 ) ,心肌梗死范围缩小 ,与对照组比较差异有显著性意义 (P <0 .0 5 ) ,LVSP ,+Pmax ,收缩功能 [L(1+2 ) ]、-Pmax、舒张功能 [L(3+4) ]与对照组比较差异有显著性意义 (P <0 .0 5 )。再灌注 6周用药组和对照组间死亡率差异有显著性意义 (P <0 .0 5 )。LVSP ,+Pmax和L(1+2 )与对照组比较差异有显著性意义 (P <0 .0 5 )。而LVEDP ,-Pmax、L(3+4)与对照组比较差异无显著性意义 (P >0 .0 5 )。结论 抗ICAM Objective To evaluate the effect of anti intercellular adhesion molecule 1(ICAM 1) monoclonal antibody on cardiac function in rats with myocardial ischemia and different phase of reperfusion.Methods Anesthetized rats were subjected to total occlusion (45 min) of the left main coronary artery followed by 24 h and 6 weeks reperfusion.The area of myocardial necrosis,and mortality were observed.The index of myocardial contractive LVSP,+Pmax,L(1+2) and diastolic function LVEDP,-Pmax,L(3+4) were evaluated by hemodynamic analytic apparatus.Results By 24 h of reperfusion,there was no significant difference in mortality between control group and anti ICAM 1 MAb treated one.The size of the infarct area was significantly lower in treated group(P<0.05).The value of LVSP,+Pmax,L(1+2),LVEDP,-Pmax and L(3+4) in treated rats were increased significantly compared with that in control ones(P<0.05).By 6 weeks of reperfusion,the mortality in treated group was significantly lower than that in control one(P<0.05).The value of LVSP,+Pmax and L(1+2) in treated rats were significantly different from that in control ones(P<0.05).However,the value of LVEDP and -Pmax were similar between the two groups.Conclusions These data indicated that anti ICAM 1 MAb has a potential effect on reducing the mortality and improving cardiac function in rats with early or long term reperfusion after myocardial ischemia.
