多囊卵巢综合征高雄激素血症患者瘦素水平的检测及评价

Measurement of Serum Leptin Levels in Polycystic Ovary Syndrome with Hyperandrogenemia and its Significance

目的 :探讨多囊卵巢综合征高雄激素血症进行患者瘦素 (Leptin)水平及其与睾酮 (T)、体重指数(BMI)的关系。方法 :采用放射免疫分析法 (RIA)检测 5 3例PCOS患者血清瘦素水平 ,其中非肥胖组 :PCOS患者36例 ,正常对照 30例 ;肥胖组 :PCOS患者 17例 ,正常对照 2 0例。结果 :肥胖组及非肥胖组PCOS高雄激素血症患者血清瘦素水平均明显高于相当体重指数的正常对照组 ,有显著差异 (p均 <0 0 1) ;两组瘦素水平分别与其睾酮水平进行相关性分析 ,均显著正相关 (r=0 5 1,p <0 0 1和r =0 5 8,p <0 0 1) ;非肥胖组PCOS患者血清瘦素水平与其BMI相关分析显示 ,无相关性 ;肥胖组PCOS患者血清瘦素水平与其BMI进行相关分析 ,显著正相关 (p =0 5 6 ,p<0 0 1)。结论 :高瘦素血症是PCOS患者的内分泌特征之一 ,PCOS高雄激素血症的形成与高瘦素水平有关 ;超重或肥胖的PCOS高雄激素血症患者存在瘦素抵抗现象。

Objective To investigate the serum leptin concentration in polycystic ovary syndrome (PCOS) with hyperandrogenemia (HA) and its relationship with testosterone (T) level and body mass index (BMI). Methods Serum leptin levels were measured by radioimmunoassay in 53 patients with PCOS, in which 36 cases were of the in non-obesiety group and 17 cases in obese group. 30 non-obese cases and 20 obece cases without PCOS served as controls. Results The levels of leptin in PCOS with HA in obese and non-obese group were significantly higher than those in control group (all p <0 01). There was a positive correlation between serum leptin levets and tesntosterone levels in both groups ( r=0 51,p <0 01 and r=0 58,p <0 01),No correlation existed between serum lepin levels and body mass index (BMI) in PCOS patients of non-obese group but there was positive correlation in obesity group ( r=0 56,p <0 01). Conclusion High serum leptin levels is one of the characteristic in PCOS patients, The HA in PCOS patients is related to serum leptin levels and hearyweight or olbese patients have the phenomenon of leptin resistance.
