血清CA125在肝硬化与肝癌诊断中的价值

Value of serum CA125 in diagnosis of liver carcinoma and hepatocirrhosis

目的探讨血清CA125在肝硬化与肝癌诊断中的价值。方法回顾分析42例肝硬化与53例肝癌患者及120例正常对照组的CA125水平。结果肝癌组、肝硬化组CA125分别为312.03±214.45U/ml、288.75±185.47U/ml;以35U/ml为界限值,肝癌组阳性率为64.1%,肝硬化组阳性率为59.5%。均显著高于正常对照组(P<0.01),但肝硬化组与肝癌组两者CA125水平及阳性率均无显著性差异(P>0.05)。肝癌组AFP水平及阳性率均显著高于肝硬化组与正常对照组(P<0.01)。AFP结合CA125联检肝癌的敏感性可提高到88.7%。结论CA125可作为肝硬化与肝癌的辅助诊断指标。如能结合AFP检测,可在两者的鉴别诊断方面及肝癌诊断的敏感性上得到进一步的提高。

Objective To investigate the diagnostic value of serum carbohydrate antigen125(CA125)in liver carcinoma and hepatocirrhosis.Methods We reviewed the serum CA125levels of42patients with hepatocirrhosis,53patients with liver carcinoma and120healthy persons as normal control.Results The average level of serum CA125was312.03±214.45U/ml in liver carcinoma and288.75±185.47U/ml in hepatocirrhosis,When35U/ml was taken as the cutoff value,the positive rate of serum CA125was64.1%in liver carcinoma and59.5%in hepatocirrhosis.The average level and the posi-tive rate of serum CA125in liver carcinoma and in hepatocirrhosis were significantly higher than these in normal control(P<0.01),but the difference of them between in liver carcinoma and in hepatocirrhosis was not significant (P>0.05).The average level and the positive rate of serum AFP in liver carcinoma was significantly higher than in hepatocir-rhosis and normal control(P<0.01).The combining measurement of CA125and AFP can raise the diagnostic sensitiveness of liver carcinoma to88.7%.Conclusion CA125can be used as an accessory parameter in the diagnosis of liver carcinoma and hepatocirrho-sis.If CA125and AFP are combined for detectin,the differential diagnosis of them and the diagnostic sensitivity of liver carcinoma can be further improved.
