直肠印戒细胞癌的临床及病理学探讨

Clinic and pathological study of signet ring cell carcinoma of the rectum

目的 探讨直肠印戒细胞癌的临床及病理特点。方法 回顾性地分析 4 4例直肠印戒细胞癌的临床和病理资料。结果 本组病例平均年龄 38.2 (2 8～ 6 8)岁。入院前误诊率 6 5 % ,平均误诊时间 8.2个月。DucksC期和D期占 85 % ,根治性手术切除率仅 5 9.1% ;术后复发率为 34.6 %。 5年生存率为 4 .8%。主要死亡原因为广泛的腹腔种植性转移 (88.1% )。结论 直肠印戒细胞癌误诊率高、恶性度高、进展速度快、预后不良。

Objective To study the clinic and pathological character of signet ring cell carcinoma of the rectum. Methods To retrospectively analysis clinical and pathological data of 44 signet ring cell carcinoma of the rectum in my hospital. Results Average age in the group was 38.2 years old. The misdiagnosis rate of re hospitalizations was 65%, and the average misdiagnoses time was 8.2 months. All of the Ducks staging C and Ducks staging D tumors account for 85%. The radical curative resection was only 59.1%. The postoperative recurrent rate was 34.6%. The main dead reason of the patients was extensive peritoneum metastasis ( 88.1%) and 5 year survival rate accounts only for 4.8%.Conclusion The signet ring cell carcinoma of the rectum is a kind of high malignancy and mal prognosis tumor. Preoperative final diagnosis has very important clinical value. It is necessary that more extensive radical curative resection for the signet ring cell carcinoma of the rectum than for adenocarcinoma of rectum.
