老年性系统性红斑狼疮的临床特征

CLINICAL FEATURE OF SYSTEMIC LUPUS ERYTHEMATOSUS IN THE ELDERLY

目的 探讨老年性系统性红斑狼疮患者的临床特征。方法 总结 5 6例老年性系统性红斑狼疮患者临床表现、自身抗体分布特点及治疗效果 ,并与中青年患者对比分析。结果 老年组蝶形红斑、盘状红斑、光过敏、脱发、雷诺现象等皮肤表现及肾损害和神经系统损害较中青年组明显少见 (P <0 .0 5 ) ,肺部病变、肌痛和肌无力、疲乏明显高于中青年组 ,而血液系统损害和狼疮的两项特异性抗体 (抗ds-DNA和抗Sm抗体 )、非特异性抗体 (抗SS -A和抗SS -B抗体 )以及补体C3 下降的阳性率都低于中青年组 (P <0 .0 5 )。结论 老年性与中青年性系统性红斑狼疮相比在临床表现、实验室检查等方面有其自身特点 ,在临床诊断和治疗中应给予足够重视。

Objective\ To observe the clinical feature of the elderly patients with systemic lupus erythematosus (SLE).Methods\ Sum up clinical manifestation,distribution feature of autoantibody and effect of treatment of the old with SLE, compared with middle-youth-aged.Results\ Butterfly-wings erythema,discoid erythema,light allergy,trichonhea,raynaud's phenomenon etc.of skin expression and damage in kidney and nerval system in the old group were obviously less than middle-youth-aged group (P<0.05).Lung damage,muscular pain,weakness of the muscle and fatigue were obviously higher than middle-youth-aged group,but the damage blood system and the positive rate of two specific antibodies (anti ds-DNA and anti-smith antibody),nonspecific antibodies (anti SS-A and anti SS-B antibody) and the decrement of complement C 3 were obviously lower than middle-youth-aged group(P<0.05).Conclusion\ The elderly and younger age group with SLE have their own features as for clinical manifestations and laboratory examinations.
