高血压合并大血管病变患者的脉压和胰岛素抵抗

Pulse Pressure and Insulin Resistance in Hypertensive Patients with Large Vascular Disease

目的 探讨脉压、胰岛素抵抗在高血压、高血压合并大血管病变中的作用。方法 对 31例正常对照者、36例单纯高血压患者、4 0例高血压合并大血管病变患者测量血压、脉压 ,测定空腹血糖(FPG) ,胰岛素 (FINS) ,计算胰岛素抵抗指数 (IR =FPG FINS/ 2 2 .5 )。结果 高血压合并大血管病变组与单纯高血压组和对照组比较 ,脉压在高血压合并大血管病变组最高 (P分别 <0 0 1和 <0 0 5 )。虽然单纯高血压组和高血压合并大血管病变组IR高于对照组 (P <0 0 5 ) ,但IR在高血压二组间无显著性差异。结论 在高血压病人出现大血管病变过程中 ,脉压的作用要强于胰岛素抵抗。因此 ,降压治疗时应注意脉压的变化。

Objective To investigate changes in pulse pressure(PP) and insulin resistance (IR) in hypertensive patients with large vascular disease(LVD). Methods PP?fasting blood glucose(FG) and serum insulin(FINS) were measured in 31 healthy control group, 36 hypertensive patients without complications (EH group) and 40 hypertensive patients with LVD(LVD group). IR was calculated using following formula. IR=FPG×FINS/22.5. Results Compared with control group and EH group, PP was the highest in LVD group( P <0 01 and P <0 05, respectively). IR was higher in two hypertensive groups than in control( P <0 05) while there was no difference between the two hypertension groups. Conclusion In the development of large vascular disease in hypertensives, the role of PP might be more important than that of IR. Therefore, changes of PP might be paid attention in antihypertensive treatment.
