癌性胸腹水的局部热化治疗

Treatment of malignantic dropsy of thoracic cavity or abdominal cavity with infusion chemotherapy and local hyperthermia

目的 观察胸腹腔灌注化疗加局部热疗对癌性胸、腹水的疗效。方法 胸腹腔灌注用药为顺铂、卡铂或草酸铂,每周1次,剂量为标准全身化疗剂量的50％。热疗用深圳先科生产的RS-1000热疗仪,每周2～3次,每次热疗60～120 min,热场中心温度42℃～43℃。结果 治疗总有效率为81.5％,其中癌性胸水完全消除率为75.3％,癌性腹水减少率为84.4％。不良反应:皮下脂肪硬结14.3％,胸闷、气促10.7％,口干、乏力23.3％。结论 胸腹腔内灌注化疗加局部热疗对癌性胸、腹水有良好的治疗作用,大部分患者可耐受局部热化治疗。

Objective To observe the efficacy of infusion chemotherapy and local hyperthermia to 56 patients suffered from malignantic dropsy of thoracic cavity or abdominal cavity. Methods Chemotherapy drug include DDP,CBP,L-OHP.The equipment of hyperthermia is RS-1000 produced by the ShenZhen XianCo corporation. Each patient receive 3-12 times hyperthermia, single treatment time last 60-120 minutes. Therapy temperature was 421 - 431. Results Overall response rate was 81.5% . Complete disappearance of pleura! fluid were found in 75% patients and decrease of ascites were found in 84.4% patients. Side-effect include hardening of fat (14.3 % ) , panting( 10.7 % ), thirsty or inertia ( 23.3 % ) . Conclusion Local infusion chemotherapy and hyperthermia is a useful treatment to malignantic dropsy of thoracic cavity or abdominal cavity, majority of the advance cancer patient can tolerance to local hyperthermia.
