摘要
目的 :研究FAB分型及免疫表型在过氧化物酶染色 (POX)阴性急性白血病 (AL)诊断中的诊断价值。方法 :先取POX阴性AL住院患者 ,选择符合条件的病例 ,骨髓涂片经瑞氏染色后分类计数 ,并经POX、PAS、CE、AE +NaF、NAP积分等细胞化学染色检查 ;免疫表型采用流式细胞仪测定 ;根据EGIL之积分标准诊断杂合型急性白血病。结果 :3 8例POX阴性AL患者 ,FAB分型诊断为ALL 3 3例 ,AML 5例 ;免疫表型诊断为ALL 2 8例 ,AML 6例 ,HAL 4例。结论 :POX不能作为区分AML和ALL的惟一凭据 ,对POX阴性的AL ,有必要予CE、AE +NaF、NAP积分等其他化学染色结合免疫表型检查 ,才能提高诊断的准确性 。
Objective:To study the value of FAB classification and immunophenotype to diagnose the acute leukemia with negative reaction of POX.Methods:Data of the acute leukemia with negative reaction of POX were collected and cases that conform to the condition were selected. The methods of myelogram are wright's stain and cytochemistry tests such as POX,PAS,CE,AE+NaF and NAP. Immunophenotypes were detected by flow cytometry. HAL was diagnosed according to EGIL scoring system.Results:38 AL patients with negative reaction of POX,33 cases were ALL,5 cases were AML typed by FAB,28 cases were ALL,6 cases were AML,4 cases were HAL by immunology typing method.Conclusion:POX can not be used as the only evidence to differentiate AML from ALL. It is necessary to use cytochemistry tests such as CE,AE+NaF,NAP and immunology typing method for patients with AL with negative reaction of POX,so that the diagnosis rate can be raised and misdiagnosis will be avoided.
