家兔全氟异丁烯吸入急性肺损伤模型的建立 被引量:3

Establishment of Perfluoroisobutylene-induced Acute Lung Injury Model of Rabbits

摘要 [目的 ]建立家兔全氟异丁烯 (perfluoroisobutylene,PFIB)吸入急性肺损伤 (acutelunginjury ,ALI)模型。 [方法 ]家兔麻醉后气管切开并插管 ,在已有的大、小鼠全身暴露动态染毒装置尾端串联呼吸机 ,呼吸机连接气管插管后对兔通过机械通气染毒。 2 0只家兔按染毒时间 0、1 0、2 0、2 7、33min(染毒浓度 0 3mg/L)随机分为对照组和实验组Ⅰ、Ⅱ、Ⅲ、Ⅳ ,每组 4只。连续监测染毒前后心率、血压、呼吸频率和PaO2 ,PaCO2 ;1 2h采血检测血浆总蛋白浓度后处死动物 ,检测支气管肺泡灌洗液 (bronchoalveolarlavagefluid,BALF)的蛋白浓度 ,计算肺毛细血管通透系数 ;剪取右肺上叶称湿、干重 ,记算肺湿干重比 ;取右下肺缘组织行病理学检查。 [结果 ]①各实验组染毒期间混合室各时点PFIB平均浓度与染毒起始浓度相比无统计学差异 (P >0 0 5) ;②实验组Ⅲ 8h和实验组Ⅳ 4h与对照组相比PaO2 开始显著下降、呼吸频率显著增加 (P <0 0 5) ;③实验组Ⅱ、Ⅲ、ⅣBALF中总蛋白浓度、肺湿干重比、肺毛细血管通透系数与对照组相比显著升高 (P <0 0 5) ;④实验组Ⅳ动物在染毒后 9h内全部死亡。 [结论 ]家兔经呼吸机染毒混合室PFIB浓度可保持相对稳定。PFIB浓度 0 3mg/L ,染毒时间在 2 0~ 2 7min范围内时 ,动物染毒后 1 To establish rabbit model of PFIB induced acute lung injury(ALI). We operated tracheotomy and trachealcannula on the anesthetic rabbits,jointed the rat dynamic exposure system to a breathing apparatus and cascaded it to the trachealcannula and then do machine ventilation. 20 rabbits were divided into 5 groups at random according to the exposure time for 0,10,20,27 or 33 min,the exposure concentration was 0 3 mg/L. We continuously monitored respiratory rate,heart rate,blood pressure and blood gas analysis. The rabbits were killed at 12 h after exposure and calculated the protein concentration of BALF,wet/dry ratio and the blood capillary permeability of lung. Pathologic examination was done with right inferior lobe. (1)The exposure concentration of PFIB was stable during the period of exposure ( P >0 05). (2)The PaO 2 began to decrease and respiratory frequency began to increase notably in Group Ⅲ after 8 h and groupⅣ after 4 h respectively ( P <0 05). (3)The parameters:total protein concentration of BALF,wet/dry ratio,and the blood capillary permeability of lung,raised strikingly in groupⅡ,Ⅲ and Ⅳ compared with control group respectively ( P <0 05). (4)All rabbits in group Ⅳ died within 9 h after exposure. [Conclusion] The exposure concentration of PFIB in poisonous gas mixture room is relatively stable at least for 33 minutes. The suitable exposure time of rabbit is 20~27 min at exposure concentration of 0 3 mg/L,between which the physiological state is relatively stable during 10 hours and the protein concentration of BALF,wet/dry ratio and the blood capillary permeability of lung are sensitive to the degree of ALI,while blood gas analysis not.
