嗜酸粒细胞性中耳炎的临床诊治

Clinical diagnosis and treatment of eosinophilic otitis media

目的:探讨嗜酸粒细胞性中耳炎(EOM)的临床表现和诊治经验。方法:回顾性分析3例EOM患者的病史、临床表现、相关辅助检查。结合文献讨论EOM临床特征、诊断和治疗。结果:3例EOM患者患耳临床特点可以归纳为:大量黄白色分泌物或息肉形成,耳痒症状明显;息肉活检示大量嗜酸粒细胞浸润;局部使用含有糖皮质激素的滴耳液治疗有效。结论:EOM是慢性中耳炎的新的疾病类型,其具有特征性临床表现,治疗上给予以糖皮质激素为主的综合性治疗。

Objective:Eosinophilic otitis media(EOM)is a rare,refractory otitis media.This article summarizes the clinical manifestations and diagnosis and treatment experience of EOM.Method:Retrospective analysis of 3 cases of EOM patients with medical history,clinical manifestations,and related auxiliary examinations.Discuss the EOM clinical features,diagnosis and treatment in conjunction with the literature.Result:The clinical features of 3 patients with EOM were summarized as:a large amount of yellowish white secretions or polyps formation,obvious itching symptoms;polyp biopsy showed a large amount of eosinophil infiltration;topical use of hormonecontaining ear drops treatment is effective.Conclusion:EOM is a new type of chronic otitis media.It has characteristic clinical manifestations,a comprehensive treatment based on glucocorticoids should be given.
