磁共振弹性成像在布加综合征患者介入治疗效果评估中的应用价值

Application Value of Magnetic Resonance Elastography in Evaluation of Interventional Therapy for Budd-Chiari Syndrome Patients

目的探讨磁共振弹性成像(MRE)在布加综合征(BCS)患者介入治疗效果评估中的应用价值。方法搜集2016年6月至2017年11月徐州医科大学附属医院45例进行MRE测量肝脏硬度(LS)受试者的临床资料,包括30例BCS患者和15名健康受试者。30例BCS患者均进行介入手术治疗,其中12例患者术前、术后分别进行MRE的检查,18例患者只在术前进行1次MRE检查。健康受试者与BCS患者的LS值比较采用两独立样本的非参数检验,BCS患者的术前、术后LS值采用配对样本的非参数检验。结果健康受试者的LS值为2.12(1.94,2.67)kPa,BCS患者介入术前的LS值为6.36(4.70,7.40)kPa,两者之间差异具有统计学意义(P<0.001)。BCS患者的LS远远高于健康受试者,与此同时,患者在进行介入手术治疗后,血管恢复通畅,其LS值降低至4.34(3.25,5.67)kPa,术前、术后LS值差异具有统计学意义(P=0.037)。结论 MRE在BCS患者的介入手术疗效评估中有着一定的价值,测量其手术前后的LS值,可以用来评估介入手术的疗效。

Objective To explore the role of magnetic resonance elastography(MRE) in assessing the effect of interventional treatment in Budd-Chiari syndrome(BCS) patients. Methods A total of 45 individuals underwent MRE to measure liver stiffness(LS) at our institution from June 2016 to November 2017,including 30 BCS patients and 15 healthy people.All 30 BCS patients treated with interventional treatment,12 of them were examined by MRE before and after operation,the remaining 18 patients only underwent one MRE examination before operation.We used the Two Independent Samples Tests to evaluate liver stiffness value between healthy individuals and BCS patients,and using Two Related Samples to evaluate LS value between the preoperative and the postoperative. Results Healthy people,lS is 2.12(1.94,2.67) kPa,the LS in BCS patients before intervention was 6.36(4.70,7.40) kPa,there was a significant difference between them(P<0.001),at the same time,and the LS was significantly reduced to 4.34(3.25,5.67) kPa of patients after the surgery,there was a significant difference between them(P=0.037). Conclusion MRE has a certain value in the evaluation of interventional effect in patients with BCS.Measuring the LS before and after surgery can be used to evaluate the effect of interventional surgery.
