摘要
目的探讨MRI纹理分析在垂体腺瘤质地评估中的应用价值。方法回顾性分析经手术及病理证实的垂体腺瘤患者106例,所有患者术前均行MRI平扫。以手术中外科医师对瘤体质地的评估为标准分为质地软组与质地韧组,应用纹理分析软件在T_1WI、T_2WI上逐层勾画肿瘤边界,统计得到整个瘤体的纹理分析相关参数:均数、熵值、偏度、峰度及体积。采用独立样本t检验对两组间纹理分析参数进行比较,对熵值与偏数两组参数进行ROC曲线分析,分别计算曲线下面积、最佳截断点、敏感性及特异性。结果质地软组89例,质地韧组17例。在T_2WI上,两组间熵值分别为5.33±0.21、5.17±0.22,偏度分别为-0.32±0.68、0.27±0.64,差异具有统计学意义(P<0.05),熵值及偏度的最佳截断点分别为5.205、0.5085;而均数、峰度及体积在两组间差异无统计学意义(P>0.05)。两组间比较T_1WI纹理分析各参数差异均无统计学意义。结论术前MRI平扫T_2WI纹理分析在垂体腺瘤患者瘤体质地的评估中有较高的应用价值。
Objective To investigate the value of MRI texture analysis in the evaluation of pituitary adenoma texture.Methods A retrospective analysis of 106 patients with pituitary adenoma confirmed by operation and pathology was performed.All patients underwent MRI scan before operation.The evaluation of the texture of the tumor by the surgeon was divided into the soft tissue group and the texture toughness group.The texture analysis software was used to delineate the tumor boundary layer by layer on T1 WI and T2 WI,and the parameters related to texture analysis of the whole tumor were statistically obtained:Number,entropy,skewness,kurtosis and volume.The independent sample t test was used to compare the texture analysis parameters between the two groups.The ROC curve analysis was carried out on the entropy and partial parameters,and the area under the curve,the optimal cut-off point,sensitivity and specificity were calculated.Results There were 89 cases of soft tissue group and 17 cases of texture tough group.On T2 WI,the entropy values were5.33±0.21 and 5.17±0.22,respectively,and the skewness was-0.32±0.68 and 0.27±0.64,respectively.The difference was statistically significant(P<0.05),and the entropy and skewness were the most.The good cut-off points were 5.205 and 0.5085,respectively.The mean,kurtosis and volume were not significantly different between the two groups(P>0.05).There was no significant difference in the parameters of T1 WI texture analysis between the two groups.Conclusion Preoperative MRI plain T2 WI texture analysis has a high value in the evaluation of the tumor texture of pituitary adenoma patients.
