磁共振梯度多回波R_2~*成像在直肠癌术前分期分级中的应用 被引量:4

The Application of Magnetic Resonance Gradient Multi Echo R_2~* Imaging in Preoperative Staging and Grading of Rectal Cancer

摘要 目的探讨磁共振梯度多回波R_2~*成像在直肠癌术前分期分级中的诊断价值。方法回顾性分析47例直肠癌患者的MRI资料,分别测量直肠癌各分期分级的平均R_2值,采用单因素方差分析及独立样本t检验的方法,分析其统计学差异。结果各组研究对象的平均R_2值分别为T1+2期(47.4589±24.8612)Hz,T3期(66.6767±23.2849)Hz,T4期(45.8760±27.5860)Hz。T3期与T1+2期、T4期比较,差异有统计学意义,T1+2期与T4期比较差异无统计学意义。无淋巴结转移组平均R_2值为(61.8931±28.7860)Hz,有淋巴结转移组中平均R_2值为(51.1743±20.5840)Hz,差异无统计学意义。高分化组平均R_2值为(26.7467±1.4814)Hz,中分化组平均R_2值为(64.9168±24.1758)Hz,低分化组平均R_2值为(49.4545±23.2327)Hz,差异具有统计学意义。结论磁共振梯度多回波R_2~*成像有助于鉴别诊断直肠癌术前T分期及病理分级,但是对于直肠癌术前N分期价值不大,仍需进一步研究。 Objective To investigate the diagnostic value of magnetic resonance gradient multi-echo R2*imaging in preoperative staging of rectal cancer.Methods The MRI data of 47 patients with rectal cancer were retrospectively analyzed.The mean R2 values of each stage of rectal cancer were measured.The statistical differences were analyzed by one-way analysis of variance and independent sample T test.Results The average R2 values of the subjects in each group were T1+2(47.4589±24.8612)Hz,T3(66.6767±23.2849)Hz,and T4(45.8760±27.5860)Hz.There was a statistically significant difference between T3 and T1+2 and T4.There was no significant difference between T1+2 and T4.The mean R2 value of the no lymph node metastasis group was(61.8931±28.7860)Hz,and the mean R2 value(51.1743±20.5840)Hz in the lymph node metastasis group was not statistically significant.The average R2 value of the well-differentiated group was(26.7467±1.4814)Hz,the average R2 value of the moderately differentiated group was(64.9168±24.1758)Hz,and the average R2 value of the poorly differentiated group was(49.4545±23.2327)Hz.The difference was statistically significant.Conclusion Magnetic resonance gradient multi-echo R2*imaging is helpful for the differential diagnosis of preoperative T stage and pathological grade of rectal cancer,but it is still worthy of further study for the preoperative N stage of rectal cancer.
