摘要
目的应用静息态功能磁共振成像(rs-fMRI)技术,从神经影像学的角度研究注意缺陷多动障碍(ADHD)儿童认知发展的特征。方法 45例ADHD儿童按年龄分为3组(7~8岁、9~10岁、11~12岁各15例)进行rsf MRI扫描,采用局部一致性(Re Ho)作为测量指标。结果三组间差异显著的脑区位于小脑后叶(F=13. 751,P=0. 000)、左额中回(F=9. 795,P=0. 001)、楔前叶(F=15. 624,P=0. 000)、左额上回(F=17. 953,P=0. 000);两组间比较在小脑后叶和楔前叶,7~8岁组Re Ho值高于9~10岁组和11~12岁组差异均有统计学意义(P <0. 05);左额中回7~8岁组和9~10岁组Re Ho值均高于11~12岁组差异有统计学意义(P <0. 05);左额上回9~10岁组和11~12岁组Re Ho值均高于7~8岁组差异有统计学意义(P <0. 05)。结论 ADHD儿童的认知发展既存在发展延迟也存在偏离正常轨迹的情况,局部一致性方法为ADHD儿童早期干预提供了一种客观依据。
Objective To explore the characteristics of cognitive development in children with attention deficit hyperactivity disorder(ADHD)using resting-state functional magnetic resonance imaging(rs-fMRI)from the perspective of neuroimaging.Methods Forty-five children with ADHD were divided into three groups according to their ages(fifteen each aged 7-8,9-10 and 11-12)and received the rs-fMRI scans.A regional homogeneity(Re Ho)approach was used to analyze f MRI datas.Results There were clear differences among the three groups in posterior cerbellar lobe(F=13.751,P=0.000),left middle frontal gyrus(F=9.795,P=0.001),precuneus(F=15.624,P=0.000)and left superior frontal gyrus(F=17.953,P=0.000).Compared to the groups of 9-10 and 11-12 years old,the group of 7-8 years old showed higher Re Ho in posterior cerbellar lobe and precuneus,and there were statistically significant differences(P<0.05).In left middle frontal gyrus,the Re Ho values of the groups of 7-8 and 9-10 years old were higher than those of the group of 11-12 years old and there were statistically significant differences(P<0.05).Meanwhile,the groups of 9-10 and 11-12 years old showed higher Re Ho values than the group of 7-8 years old in left superior frontal gyrus and therewere statistically significant differences(P<0.05).Conclusion There exists developmental delay and deviation from the normal track in the development of children with ADHD.The approach of Re Ho offers a kind of objective evidence for the early intervention of children with ADHD.
