早发性及晚发性帕金森病大脑皮层厚度的研究 被引量:4

The Cortical Thickness in Early-onset and Late-onset Parkinson’s Disease

摘要 目的通过比较早发性帕金森病(EOPD)组与晚发性帕金森病(LOPD)组大脑皮层厚度的改变,探讨EOPD、LOPD的不同发病机制。方法搜集病程匹配的EOPD患者21例、LOPD患者22例以及年龄分别匹配的年轻健康对照组(YC)16名、年老健康对照组(OC)20名,实验组及对照组均采用3.0 T MR扫描,采集脑高分辨率3D-T1WI结构图像,利用FreeSurfer软件比较各组皮层厚度的差异。结果 EOPD组与YC组比较,EOPD组右侧额上回、中央旁小叶、顶上下小叶及楔前叶的皮层厚度增加(P<0.05),右侧梭状回、舌回皮层厚度变薄(P<0.05);LOPD组与OC组比较,LOPD组的右侧颞上回、颞中回皮层厚度变薄(P<0.05)。结论 EOPD与LOPD存在不同的脑皮层厚度改变,为以后深入研究EOPD、LOPD的发病机制提供影像学依据。 Objective We aimed to compare the cortical thickness between Early-onset Parkinson’s disease(EOPD)and late-onset Parkinson’s disease(LOPD)to investigate the possible mechanism of Parkinson’s disease(PD).Methods forty-three patients with PD and thirty-eight healthy controls were included,of which the PD group were classified into 2 subgroups on the basis of early(Onset age≤50,n=21)and late(Onset age>50,n=22)onset of PD with matching of disease duration,and healthy controls also divided into younger healthy control subgroup(YC,n=16)and older healthy control subgroup(OC,n=20)according to the matching of age with PD subgroups.All the patients underwent a 3-tesla structural MRI scans and complete neuropsychological assessment.FreeSurfer image processing and analysis software was used to measure cortical thickness across groups.Results EOPD patients showed increased cortical thickness in the right superior frontal gyrus,the superior and inferior parietal lobe,paracentral lobe and the precuneus lobe,and decreased cortical thickness in the right lingual and fusiform compared to the YC group.Cortical thinning was observed in the right superior and middle temporal gyrus in LOPD patients compared with OC.Conclusion This study found that EOPD and LOPD have different changes in cortical thickness,providing a brain imaging basis for further study of the pathogenesis of EOPD and LOPD.
