儿童甲状旁腺腺瘤的MRI表现

摘要 目的探讨儿童甲状旁腺腺瘤(PTA)的MRI表现,以提高对该病的认识。方法搜集行颈部MRI检查,且术后病理证实的儿童PTA 6例,分析总结其MRI特点。结果 6例PTA,男5例,女1例,年龄12~14岁。6例均为单发实性病灶,边缘清晰,长径1.2~4.0 cm;多数(4/6)瘤体长径是短径的2倍以上,长径大致对应于纵径、短径对应于横径。与周围正常甲状腺组织相比,T1WI瘤灶为等或稍高信号,T2WI、短时间反转恢复(STIR)及扩散加权成像(DWI)序列为略高或高信号;增强扫描实性成分明显均匀强化。结论儿童PTA多见于10岁以后,多呈纵向长椭圆形,为T2WI及DWI略高或高信号的实性结节,明显强化。 Objective To analyze and summarize the MRI features of parathyroid adenomas(PTAs)in children,and to improve the ability to diagnosis the disease.Methods We retrospectively collected and analyzed 6 cases of PTAs,which were confirmed by pathology after resection.Preoperative Cervical MRI scan was performed in all of the patients.Results The 6 cases included 1 female and 5 males,and aged from 12 years old to 14 years.All of the 6 cases had single,solid mass with clear edge.And the long diameter of the masses was from 1.2 cm to 4.0 cm.In 4 cases,the length of the tumors was more than 2 times the short diameter,and the length was roughly corresponding to the longitudinal diameter and the short diameter was corresponding to the transverse diameter.In comparison with the signal intensity of the normal thyroid gland,the PTAs showed isointensity or slight hyperintensity on T1WI and slightly high signal or high signal on T2WI,short TI inversion recovery sequence(STIR)and diffusion weighted imaging(DWI).The solid components of the tumors showed intense homogeneous enhancement after injection of contrast agent.Conclusion PTAs in children occur predominantly after 10 years of age.The main MRI features of the PTAs include lengthwise long ellipsoid,solid nodules,with slight hyperintensity or hyperintensity on T2WI and DWI,and with intense uniform enhancement.
