摘要
目的:观察牙周基础治疗后牙龈瘤上皮层及结缔组织层的组织学变化。方法:收集临床诊断为血管性龈瘤患者30例,随机分为两组,每组各15例;对照组初诊时手术切除血管性龈瘤;实验组经牙周基础治疗后2~3个月手术切除血管性龈瘤。检测两组切除组织HE染色后上皮厚度,结缔组织微血管密度计数和结缔组织胶原纤维面积。结果:未行牙周治疗的对照组和经牙周基础治疗后2~3个月的实验组,上皮厚度分别为(277. 9±23. 39)μm,(143. 2±10. 05)μm,两组间差异有统计学意义(P<0. 05);微血管密度计数分别为(70. 77±5. 08)个和(14. 15±2. 43)个,两组间差异有统计学意义(P<0. 05);结缔组织层胶原纤维面积分别为(23. 64±3. 49)%和(44. 16±3. 80)%,两组间差异有统计学意义(P<0. 05)。结论:牙周基础治疗后,血管性龈瘤上皮厚度减少,结缔组织层微血管密度计数下降,胶原纤维面积升高。
Objective: To evaluate the histological changes of the epithelium and connective tissue of epulis granulomatosa after periodental initial therapy. Methods: Thirty patients cases which diagnosed the epulis granulomatosa were randomly devided into two groups,with 15 patients in each group. Surgical excision was performed in the control group,and underwent surgical resection after 2 ~ 3 months of initial therapy in experimental group. The changes in the epithelium width and epithelial cells after HE staining,the microvessel density and the area ratio of the collagen fibres in connective tissue were analysed. Results: After periodental initial therapy,the epithelial width gradually decreased from( 277. 9±23. 39) μm to( 143. 2 ±10. 05) μm,there was statistically significant difference between two groups. The microvessel density gradually decreased from( 70. 77±5. 08) to( 14. 15±2. 43) ,while there was a gradual increase from( 23. 64±3. 49) % to( 44. 16±3. 80) % in the area of collagen fibers,there was statistically significance difference between two groups,respectively. Conclusion: The microvessel density of epulis granulomatosa group gradually decreased when collagen fiber area gradually increased. The restoration of epithelial histology provides a theoretical basis in the therapy of epulis.
