两种不同材质临时基台的前牙区即刻种植修复临床效果

Clinical evaluation of two temporary abutments in immediate dental implant and restoration

目的:观察分析前牙区两种不同材质临时基台在拔牙后即刻种植即刻修复的临床效果。方法:选取前牙缺牙区拔牙后即刻种植修复患者30例,共74枚种植体;并采用随机方式分别放置塑料制或金属制临时愈合基台,其中50枚(实验组)采取接入塑料制临时愈合基台,24枚(对照组)采取接入金属制临时愈合基台;术中同时即刻行复合树脂临时冠/桥修复。术后根据患者骨结合情况,种植术后6个月,更换永久性美学基台行固定修复。对比两组种植体在种植术后1年、2年和3年的种植体周围牙槽骨吸收量及患者满意度。结果:74枚种植体均成功存活且稳定性良好,牙槽骨吸收量与患者满意度的评比分析上,塑料制临时愈合基台较金属制佳。结论:严格掌握好病例选择适应症,前牙区拔牙后即刻种植即刻修复,接入塑料制临时基台制作树脂临时冠种植术后牙槽骨吸收量稳定,且具有较高的美学修复效果及患者满意度。

Objective: To observe and analyze the clinical effects of immediate restoration of two kinds of temporary abutments in the anterior area. Methods: For 30 cases( total 74 implants),the implants were implanted immediately after tooth extraction in the edentulous area of the anterior teeth. At the same time,two different kinds of temporary abutments of plastic and metal were used by random method. Among them,50 were connected to the plastic temporary abutment,and 24 were connected to the metal temporary abutment. During the surgery,the composite resin temporary crown/bridge repair was immediately performed. According to the patient’s osseointegration condition,the temporary abutment was replaced to the permanent aesthetic abutment at the 6 thmonth after implantation,and the alveolar bone resorption and patient satisfaction were compared in one,two and three years after implanting. Results: All 74 implants were successful and had good stability.In the evaluation of alveolar bone resorption,gingival aesthetics and patient satisfaction,the plastic temporary abutment had better result than the metal temporary abutment. Conclusion: It is important to choose the right indication. After that,the plastic temporary abutment is used to make the temporary resin crown which will have a stable the alveolar bone absorption,obtain high aesthetic restoration effect and patient satisfaction.
