血管内超声确诊右编织样冠状动脉1例

One case of woven right coronary artery diagnosed by intravenous ultrasound

1病例资料患者,男,48岁,因“间断咳嗽10年,再发伴胸闷3个月余”于2018年7月2日入院。10年前无明显诱因出现间断咳嗽,平卧休息时为主,坐起可缓解,于当地医院查心脏彩超提示心脏扩大,诊断“冠心病,心功能不全”,住院治疗好转,此后不规律.

Summary Woven coronary artery is a rare congenital anomaly coronary artery disease with an unknown etiology.A 48-year-old male presented with intermittent cough for ten years and then showed chest tightness for three months.ECG characterized by abnormal Q wave inⅡ,Ⅲ,avF and V1-V6.BNP was 342 pg/ml.Right coronary angiography showed a twisting course of the right coronary artery lumen after it divided into multiple thin channels.Due to the special contrast image and suspected woven changes,intravenous ultrasound(IVUS)examination was employed and showed that the proximal coronary artery was divided into two smaller lumens and extended to 3-4 lumens.A diagnosis of woven right coronary artery was considered,and the patient was discharged with medical therapy.
