北京肿瘤医院静脉药物配置中心用药错误及干预效果分析

Drug misuse and intervention analysis in intravenous drug allocation center of Beijing Cancer Hospital

目的:分析北京肿瘤医院(以下简称我院)静脉药物配置中心用药错误,制定干预措施,进而评价干预效果。方法:统计我院静脉药物配置中心2017年与2018年发生用药错误时间分布和错误类型,分析常出现的操作失误及高频出错规律。结果:2017年我院静脉药物配置中心调配医嘱80 215条,发生用药错误227例,错误发生率0.28%。错误类型包括调配错误、提交错误、配送错误,调配错误发生的次数最多,共计114例,占50.22%。2018年共调配医嘱101 548条,发生用药错误147例,错误发生率0.14%,较2017年错误发生率下降了0.14%,差异有统计学意义(P<0.05)。结论:通过对用药错误事件的回溯性分析,可有的放矢地完善管理措施,加强用药安全管理。

Objective:To analyze the medication errors in the intravenous drug allocation center in Beijing Cancer Hospital,so as to establish intervention measures and evaluate the intervention effect Methods:The time distribution and medication errors types in our hospital’s intravenous drug allocation center in 2017 and 2018 was statistical analysed.The regularity of frequent operation errors and high frequency errors was found out Results:In 2017,there were 80 215 dispensing prescription and 227 cases of medication errors in our hospital’s intravenous drug allocation center,and the incidence of errors was 0.28%.The types of medication errors included allocation errors,submission errors,and delivery errors.The most frequent one was allocation errors,and there were totally 114 cases,accounting for 50.22%.In 2018,there were 101 548 dispensing prescription and147 cases of medication errors.The errors incidence was 0.14%,which decreased 0.14%compared with 2017,and the different was statistically significant(P<0.05).Conclusion:Through the retrospective analysis of medication errors,we can improve the management measures and strengthen the quality management of medication.
