心肌酶活性比较探讨体外与非体外循环冠脉搭桥术对心肌影响

摘要 目的 探讨体外循环与非体外循环冠状动脉搭桥术 (CCABG与OPCAB)中的心肌酶活性比较以评价两者对心肌保护的影响。方法 将 2 0例冠状动脉搭桥患者分为两组 :A组为CCABG 10例 ;B组为OPCAB 10例。手术前、手术后 2 4,48,72 ,84,96h取动脉血标本测试心肌酶AST、LDH、LDH -1、CK、CKMB、HBDH等活性进行统计学分析。结果 手术后两组血清心肌酶活性均增加 ,术后 2 4h为最高值。A组 72h内血清酶活性高于B组 (P <0 0 5 ) ,但在 84h全部酶类接近正常值 ,96h全部酶类恢复至正常水平。两组在第 84,96小时心肌酶活性差异无显著性 (P >0 0 5 )。结论 体外循环下冠状动脉搭桥术的患者心肌损伤呈可逆性 ,且恢复时间短。优点在于让外科医生有充分时间更从容地完成手术操作 。
