摘要
目的 探讨高脂血症患者血清 HDL 亚类组成和相对百分含量的变化及其与血脂水平的关系。方法采用双向电泳 -免疫印迹检测法分析 16 8例高脂血症患者血清 HDL亚类组成及相对百分含量。结果 高脂血症患者血清中前 β1 - HDL、前 β2 - HDL、HDL3c、HDL3b及 HDL3a含量显著增加 (P均 <0 .0 1或 <0 .0 5 ) ,而 HDL2 a及 HDL2 b含量显著减少 (P<0 .0 1)。高脂血症患者中男性前β1 - HDL及 HDL3b含量显著高于女性 (P<0 .0 1) ,HDL2 a及 HDL2 b含量显著低于女性 (P<0 .0 1)。正常对照者中男性前 β1 - HDL 含量显著高于女性 (P<0 .0 5 ) ,HDL2 b含量显著低于女性 (P<0 .0 5 )。高脂血症患者血清 TG含量与前β1 - HDL、前β2 - HDL、HDL3b及 HDL3a水平呈正相关 (r=0 .5 83、r=0 .196、r=0 .130及 r=0 .370 ) ,而与 HDL2 a及 HDL2 b水平呈负相关 (r=- 0 .2 93及 r=- 0 .4 5 6 ) ;TC含量与前 β1 -HDL 及 HDL3b水平呈正相关 (r=0 .348及 r=0 .2 19) ,而与 HDL2 b水平呈负相关 (r=- 0 .2 76 ) ;L DL- C含量与HDL3b水平呈正相关 (r=0 .187) ;HDL - C含量与前β1 - HDL及 HDL3a水平呈负相关 (r=- 0 .35 4及 r=- 0 .2 92 ) ,而与 HDL2 a及 HDL2 b(r=0 .2 5 4及 r=0 .2 5 6 )水平呈负相关。 TG/ HDL - C比值与前β1 -
Objective To detect the change of composition and ratio of serum HDL subclasses and explore the relationship between these changes and the plasma lipid level in patients with hyperlipidema. Methods The subclasses of serum HDL in 168 patients with hyperlipidemia were determined by two dimensional gel electrophoresis in conjunction with immunodetection method. Results Compared to the healthy controls, the pre β 1 HDL, pre β 2 HDL, HDL 3c , HDL 3b , and HDL 3a increased significantly ( P <0.01, P <0.01, P <0.05, P <0.01 and P <0.01), while the HDL 2a and HDL 2b decreased significantly ( P <0.01) in the patients with hyperlipidemia. The pre β 1 HDL and HDL 3b were significantly higher ( P <0.01) while the HDL 2a and HDL 2b were significantly lower( P <0.01)in men than in women with hyperlipidemia. The pre β 1 HDL was significantly higher ( P <0.05) while the HDL 2b was significantly lower ( P <0.05)in the male control group than in the female control group. In the patients with hyperlipidemia, the serum TG was positively correlated with pre β 1 HDL, pre β 2 HDL, HDL 3b , and HDL 3a ( r =0.583, 0.196, 0.130, and 0.370 respectively)and negatively correlated with HDL 2a and HDL 2b ( r =-0.293 and -0.456 respectively); The serum TC was positively correlated with pre β 1 HDL and HDL 3b ( r =0.348 and 0.219); The serum LDL C was positively correlated with HDL 3b ( r =0.187); The serum HDL C was negatively correlated with pre β 1 HDL and HDL 3a ( r =-0.354 and -0.292 respectively) and positively correlated with HDL 2a and HDL 2b ( r =0.254 and 0.256); The TG/HDL C ratio was positively correlated with pre β 1 HDL, pre β 2 HDL, HDL 3b , and HDL 3a ( r =0.593, 0.225, 0.123, and 0.394)and negatively correlated with HDL 2a and HDL 2b ( r =-0.317 and -0.459). Conclusion The particle of HDL in the patients with hyperlipidemia shows a general shift towards smaller size, which indicates that the maturation of HDL could be abnormal in patients with hyperlipidemia. The change of subclasses of HDL in the patients with hyperlipidemia might be associated with coronary artery diseases.
